New Patient Packet

Thank you for choosing Nephrology Specialists of Tulsa. We would like to take this opportunity to prepare you for your first appointment:

We ask that you arrive 30 minutes prior to your appointment. This allows our staff adequate time to prepare everything for our physicians.
Please complete and return the gender related new patient paperwork (attached below).
Please bring your insurance card(s) and driver's license (or photo id).
Please bring your medications, including vitamins and over the counter medications to your first appointment (we prefer medications, however, if you are uncomfortable bringing your medications please bring a medication list).
At your first appointment we will ask you to leave a urine specimen when you check-in.

Your first appointment will be approximately 45 minutes. Please confirm with our staff which office you are scheduled at.
